ATTRACT Q: Can flu vaccine cause asthma exacerbations?

Is there any evidence that in patients with asthma, seasonal flu vaccine can trigger an acute exacerbation of asthma?

The answer being:

CKS clinical guidance on seasonal influenza vaccinations (1) has a section on its use in people with asthma. This states:

“No increase in asthma-related adverse effects was reported with vaccination.”

Specifically they discuss harms of inactivated vaccines and note:

“Six studies were identified. No statistical difference was found, compared with placebo, in asthma exacerbations in the 2 weeks following influenza vaccination (two studies, n = 2306), lung function and increased use of bronchodilators (four studies, n = 4924), medical consultations (three studies, n = 5092), and new or increased oral corticosteroid use (two studies, n = 4419).”

A fuller discussion of the data is available in the Cochrane review (2) that examined the effectiveness of influenza vaccination in children (older than 2 years of age) and adults with stable asthma (search date: up to September 2007).
